We understand how frustrating it can be to do “all the right things” with diet
and exercise, yet still face stubborn cellulite dimples in the mirror!

Even our most fit clients, women who compete in fitness competitions and are at or below their “ideal weight”, battle with the appearance
of cellulite on areas such as the buttocks and thighs! The fact is that cellulite affects the vast majority of adult women to some degree,
regardless of age, body habitus, diet and exercise routine. It is a genetic, age, hormone and lifestyle related condition which is typically
frustrating because of its resistance to “do-it-yourself” interventions.

You may recall a handful of years ago when the “Fascia Blaster” hit the market, promising to rid women of their unwanted cellulite in just
4 sessions a week. It promoted “intentionally bruising the skin in order to break up fascia” and “break up fat cells”. These claims were not
based on any real science and the device inventor, who holds no degrees, promoted herself as a “pioneer in fasciology” (the latter being
also a term she invented). Add to campaigns such as this the hundreds if not thousands of topical creams on the market, each promising to
treat cellulite quickly and effectively with little or no real science research to support their claims.

Getting to the Root Causes of Cellulite

To understand what truly improves cellulite and why, you need to understand the “anatomy of cellulite”, and it’s not as hard as you may think. Understand the real contributing factors, and select interventions which have been clinically proven to impact those factors, and you are on the path to smoother skin.

Thinning Skin and Skin Laxity

As we get older, our skin loses collagen and elastin. This process begins in our late 20’s and progresses over time. Although topical creams may temporarily hydrate and reduce the appearance of skin irregularities, they do little if anything to change what is going on beneath the skin regardless of the hype and promises made in ads and on the internet. A good skincare regime is always a good idea, but Acoustic Wave Therapy (AWT) also impacts cellulite by restoring lost collagen and elastin to the skin and by enhancing circulation and fat cell metabolism.

Fat Cell Hypertrophy

Age, hormonal shifts and weight gain cause our existing fat cells to swell. These inflated cells then push up against the tough fibrous bands, worsening the appearance of cellulite. Interventions proven to shrink fat cell size are weight loss (which will be effective to a point, but will likely not eliminate the appearance) and interventions which result in Lipolysis-such as Venus Bliss. Lipolysis is the release of triglycerides from the existing fat cells, which causes them to shrink down in size. The Venus Bliss device also has applicators which permanently destroy/eliminate fat cells- a process called Apoptosis.

Fibrous Bands/ Fascia

Fibrosis is a process which affects the fibrous bands which separate the fat lobules into compartments. This process causes the bands to stiffen and shrink/retract- which results in the downward pull/dimpling we see at the skin surface. This is a key mechanism in the formation of cellulite, and one which must be targeted if a treatment is to have any type of a long lasting and meaningful effect. These bands can be "cut" by manual or laser subscision. Cellfina is a procedure specifically designed to release these bands.

Understanding Cellulite: Knowledge is Power

The first step is the evaluation of your region(s) of concern. It is not uncommon
for loose skin to be confused with cellulite, and there may be some of both going
on at the same time! Based on the evaluation of our expert nurses, we can make
recommendations regarding your best course of action.

The main treatment options at Renew MediSpa to address stubborn cellulite are

  • Venus Bliss (Fat Destruction and Skin Tightening)-combines Laser, Radiofrequency and Electromagnetic energies for non-invasive body contouring with no downtime and no activity restrictions. Requires a treatment series.
  • Acoustic Wave Therapy/AWT (also called Low Intensity Shockwave Therapy/LiSWT)- a well-established method of improving collagen content and skin thickness, improving circulation, and clinically proven to be effective in reducing the appearance of cellulite. Requires a treatment series to reduce the appearance of cellulite and smooth skin.
  • Cellfina- a minimally invasive, non-surgical procedure which uses a microblade to cut the fibrous bands that cause the cellulite dimples. Results have been shown to last years after a single procedure. Cellfina is performed twice a month at Renew by Beth Wagner, RN, a former Cellfina trainer who used to travel to physician offices teaching them how to perform this remarkable procedure. Cellfina does not tighten or smooth skin. It is important to get a pretreatment in person evaluation to determine if your issue is cellulite, crepey skin, or both. 
  • Profound RF- this is a form of micro needling with heat, which is generated by radiofrequency (RF) energy. It is often a ‘single procedure’ option for smoothing and tightening and can be used on face, neck or body areas. Profound RF is good for crepey skin. It does not release cellulite bands.

Advisory

An injectable treatment called “Qwo” was FDA approved in 2020 and first offered in the USA in late Spring 2021.

Qwo is delivered under the skin into regions of cellulite in a series of injections (commonly 3 or more sessions, 21 days apart). Qwo is
composed of collagenase, a bacteria derived enzyme which breaks down collagen. The rationale for its use is that the fibrous bands
which contribute to cellulite are composed of a large amount of collagen, and thus by attempting to dissolve them with collagenase one
can theoretically improve the appearance of the cellulite.

The problem is that collagen is also abundant in the the skin, and in the walls of blood vessels in the region treated. One cannot direct the
collagenase to “only the collagen present in the fibrous bands” and spare its effect on other important local structures. Our colleagues
who have used Qwo, and patients themselves online, are reporting extremely extensive bruising. This is due to the collagenase effect on
the blood vessel walls in the area, breaking them down and leading thus to extravasation of blood into the skin and soft tissues. This
bruising can take many months to resolve. We have also seen reported a much more difficult to treat complication called hemosiderin
staining, which likewise can be a very long term issue and can in some cases produce permanent brownish discoloration of the skin.

(If you experience hemosiderin staining, we recommend that you seek out a laser specialist who had a Q switched ND:YAG laser. This will
be a safer option that an Intense Pulsed Light (IPL) intervention.)

Due to the reported complications, Dr. Vuich has decided NOT to offer Qwo as a treatment option at Renew MediSpa.

We consider the reported side effects and adverse events to be out of the range of what we consider acceptable, and have treatment
options to offer which have not produced such dramatic and drawn-out issues as we are seeing reported with Qwo.

Is there any pre-treatment preparation needed?

Cellfina: Avoid substances which increase the likelihood of bruising, such as Aspirin, Ibuprofen and similar anti-inflammatories, Turmeric, Ginger, Garlic, Ginseng and Ginko Biloba, Vitamin E and Fish Oil. Remember that there will be bruising, and a period of time when you will be required to wear a Spanx type or similar compression garment.

Venus Bliss: We recommend that you come to your appointments well hydrated, as this helps us get the tissues up to target temperature faster.

Acoustic Wave Therapy: No particular preparation is needed prior to Acoustic Wave Therapy.

Profound RF: Avoid substances which increase the likelihood of bruising, such as Aspirin, Ibuprofen and similar anti-inflammatories, Turmeric, Ginger, Garlic, Ginseng and Ginko Biloba, Vitamin E and Fish Oil. There will be bruising, and a period of time when you will need to be attentive to a post treatment skin care regimen to facilitate healing.

Who cannot have a Cellulite Treatments performed?

  • In general, procedures are not performed on pregnant women
  • Realistic expectations are important! Some of the contributing factors for cellulite, such as age-related changes of the skin, cannot be halted. This means that at some point you will require maintenance treatment sessions to keep your skin looking smooth. Likewise, large fluctuations in weight may cause the return of a cellulite appearance even after a successful treatment course. We encourage our clients to view cellulite solutions as they would any other health and beauty regime- as something which does require some ongoing attention in order to maintain the results.

What should I expect from my consultation?

One of our experiences nurses or medical assistants will review your history and take photographs. They will review the specifics of the procedures we offer, including treatment pricing. They will then present the photos and consult information to Dr. Vuich for her input and recommendations. Within a few days you will receive a return call letting you know Dr. Vuich’s suggested approach.
