Biostimulatory fillers are those with enhanced collagen stimulating components

The most common injectable fillers are composed of hyaluronic acid (HA), a naturally occurring sugar molecule, which is chemically crosslinked in order to extend the duration of its effects. HA dermal fillers are very versatile- they can be used for everything from volumizing lips to correcting fine lines and even lifting the cheeks, and their effect is immediate. Long lasting HA fillers, such as the RHA collection, have studies showing that even at 15 months post injection most patients will still have noticeable improvement in the regions treated. The placement of an HA dermal filler does stimulate some collagen also, but not as much as the other class of dermal fillers which we call “biostimulatory”.

When considering which type of dermal filler to use, there are several factors to consider and some overall pros and cons to weigh. Many clients end up benefitting from using more than one type of dermal filler, combining both traditional HA fillers and biostimulatory fillers according to their specific needs and past experiences.

The Longest Lasting Volume Correction

The collagen response to biostimulatory dermal fillers can last for years, reducing the frequency of maintenance appointments.

Biostimulatory dermal fillers contain a component which is a more powerful collagen stimulator, and it is the collagen response to the stimulator which leads to their much longer duration of action. Examples of biostimulatory dermal fillers include Radiesse, Bellafill and Sculptra. Each of these has a different mechanism for stimulating collagen.

Radiesse

Comprised of calcium hydroxylapatite (CaHA) microspheres suspended in an aqueous gel carrier. It provides immediately noticeable volume correction, followed by collagen stimulation over time which extends the duration of the correction. Radiesse has the advantage of a larger volume per syringe- 1.5 ml of product per syringe.

Bellafill

Contains a combination of bovine collagen (which dissolves over the course of about 3 months) along with polymethyl methacrylate (PMMA) microspheres which remain long term and act as a “scaffold” onto which your own natural collagen is stimulated to form. Provides some immediate correction, but then ongoing improvement over time. Bellafill produces results which are still apparent even at 5 years. Bellafill syringes contain 0.8 ml of product per syringe.

Sculptra

Contains poly-L- lactic acid, which works with your body within the deep dermis to stimulate a collagen response. A series of 3 sessions may be needed, after which results can last up to 2 years or longer. The result is gradual, not immediate, and very natural looking. Requires 5 days of self-massage (5 minutes, 5 times per day for 5 days) to the treated region. It may take a couple of months to see the correction. Sculptra is dosed by the vial (not syringe).

thumb, the biostimulatory fillers are most often used for long term volume correction of the temporal hollows, cheeks and nasolabial folds
