Understanding scar formation is the first step.

A scar is an area of fibrous tissue that replaces normal skin after an injury. The collagen fibers in the region can be present in disorganized
bundles, rather than smooth lines. There may also be a deficit of collagen in the region of a scar. A stretch mark is a type of scar that develops
when our skin stretches or shrinks quickly. The abrupt changes cause the collagen and elastin fibers, which support our skin, to rupture.
Smooth skin is characterized by plentiful, organized collagen bundles, elastin fibers, hyaluronic acid, and fibroblasts-our collagen producing
cells.

Why do scars and stretch marks bother us?

It’s human nature to prefer surfaces that are smooth and soft when it comes to the skin. Things like scars and stretch marks draw the attention of the eye, detracting from your otherwise pleasing features. No doubt you have experienced yourself or have seen someone with, for example, significant acne scarring of the face. Your attention is drawn to the irregularities, the imperfections, because they are “standing out to the eye”. These depressions also tend to trap makeup and to cause unwanted shadowing in photographs.
Fortunately, we have multiple options available to reduce the appearance of scars and stretch marks!

4 Main Categories of
treatment

Non-surgical treatments which remodel scars tend to fall into several categories:

Mechanical Disruption

A manual process seeks to remove or disrupt the unwanted fibrous tissue. Examples: Collagen Microneedling, Subscision, Dermabrasion- all of which can be combined with PRP to enhance the tissue response further. PRP stands for Platelet Rich Plasma- this is a concentrate of your own platelets (which contain many helpful growth factors) which is prepared at the time of the appointment from a simple blood draw. This PRP can then be injected or in some cases applied topically to stimulate more healing and collagen production.

Energy Based Treatments

Heat is delivered to the tissue, often in combination with a mechanical disruption. Examples: Microneedling with Radiofrequency such as Profound RF, AGNES RF or Lutronic Genius (also RF). Also included here would be Laser procedures to remove redness (ex: DermaV laser), remove brown pigment (Spectra Laser or LaseMD), or completely resurface by vaporizing scar tissue (ex: CO2 laser/SmartSkin). *Warning: Some practices have promoted the use of "Plasma Pen" devices for the treatment of scars and stretch marks. Dr. Vuich cautions clients not to proceed with "Plasma Pen" services due to their high risk of scarring. "Plasma Pen" devices have been banned in Canada due to such complications.*

Injectables

Occasionally a scar may be amenable to injectables such as Dermal filler, PRP, Bellafill, or PDO smoothing threads. These are of much more limited benefit. Bellafill, for example, is an option only for the “rolling” type of acne scar (not the “boxcar” or “ice pick” types), and Bellafill alone is not usually sufficient but it can be combined with some of the other treatment modalities if desired. PDO smoothing threads are placed into the skin where they stimulate new collagen formation for a period of 3 months.

Growth Factors

Platelet Rich Plasma (PRP)- which is rich in growth factors- can also be combined with other modalities to further stimulate tissue remodeling, resulting in a smoother appearance. There are also some other medical grade topical solutions which may be of benefit in combination with mechanical or energy-based procedures. For example, we have special serums which we use with the Lutronic Genius treatment to further enhance skin response to the treatment.

One of our experiences nurses or medical assistants will review your history and take photographs of your specific areas of concern.

They will review some of the common treatment approaches in more depth, and/or answer questions about specific procedures you feel are most of interest to you, including treatment pricing. They will then present the photos and consult information to Dr. Vuich for her input and recommendations. Within a few days you will receive a return call letting you know Dr. Vuich’s input/recommendations.

We will help you select the best start based on our many years of experience with all of these treatment modalities.

In some instances a combination of treatment types may be suggested. Common things to consider include type and location of the scars, tolerance for downtime or activity restrictions post procedure, skin care requirements post procedure, your budget, and recent or upcoming sun exposure.

  • In general, procedures are not performed on pregnant women or on patients with a history of keloid scars.  
  • If you have used Accutane or similar oral agents, a 6 month waiting period may be required for some procedures.  
  • Some treatments cannot be performed on skin that is tanned, or if you will be getting significant sun exposure in the near future to the proposed treatment area.  
  • Some scars are too large or too advanced, and require a consult regarding surgical scar revision. We do not offer surgical services, but can assist by recommending a surgeon to you. 

There is not an “absolute” waiting period needed. Some surgeons actually start treating scars with laser and/or platelet rich plasma for example, right in the operating room on the day of the surgical procedure. You may wish to check with your surgeon for their recommendations. Surgical scars are known to remodel gradually over the course of at least a year, so some will wait that long just to “see how good it gets on its own” before intervening.

We generally do not. Keloid scars are best managed by a plastic surgeon as they will frequently need to be excised.

Some patients for some procedures will require a 4 to 6 week period of skin conditioning with topical agents such as tretinoin and/or hydroquinone prior to the actual procedure. This is most often for patients with a tendency towards hyperpigmentation due to their ethnic background- African American, Eastern European, Asian, Hispanic and Native American skin types generally need to consider preconditioning their skin prior to some procedures to avoid hyperpigmentation during their recovery process.

  • This will vary depending on scar type and extent and which procedures have been chosen. We will do our best to estimate this for you at the time of the consultation. In general, the treatment of scars or stretch marks requires a series of procedures over time. 
  • When it comes to collagen remodeling, improvement is usually evident within several weeks and continues for several months. Although each treatment has benefit, it is possible that an initial treatment will not produce enough change for you to appreciate a difference “with the naked eye”. Sometimes it takes 2 or 3 sessions to notice visible changes.  
  • We will let you know if your post procedure care requires the use of any topical agents.  
  • In the case of Smart Skin CO2 laser skin resurfacing, we provide you with a post treatment skin care kit which contains most everything you will need during the healing phase.  

There are 3 main types of acne scars- ice pick, rolling (round edges) and box car (square shaped) scars. Bellafill has some benefit primarily for the rolling type of scar- which is also a type which flattens out when you spread the skin with your fingers. The other two types do not respond to Bellafill and will more likely require laser resurfacing.

Severe Acne Scarring

  • With advanced scarring such as this, SmartSkin CO2 Laser Skin Resurfacing is generally needed, and likely more than 1 session.
  • Another option is AGNES RF microneedling- multiple sessions 3 or more months apart.
  • Other modalities may be recommended as well, such as platelet rich plasma (PRP).

Stretch Marks

  • Our DermaV Laser reduces redness and helps to flatten/soften the stretch marks (collagen remodeling, reduced hypervascularity/redness)
  • Microneedling with PRP may also be recommended, or SmartSkin CO2 laser skin resurfacing (collagen remodeling, growth factor stimulation)
  • Collagen stimulation with acoustic sound wave therapy- also known as “Low Intensity Shockwave Therapy” (increases elasticity, firmness, and blood flow)

Surgical Scars

  • DermaV Laser to reduce redness and soften the scar (collagen remodeling, reduce hypervascularity/redness)
  • Possible SmartSkin CO2 laser skin resurfacing (collagen remodeling)
  • Microneedling with platelet rich plasma (collagen remodeling, growth factor stimulation)
